MEETING REGISTRATION

There are two ways to register for the 65th Annual Meeting:

Early registration is available until October 8. 2018. After this date, you must register on site at the Annual Meeting. Please note that there will be an additional $50.00 charge for attendees registering on site.

Registration and Cancellation Policy: Cancellation requests must be submitted in writing by October 8, 2018; unfortunately, STSA is unable to honor any subsequent requests. Refunds are subject to a $50 administrative fee.

HOTEL INFORMATION & RESERVATIONS

The Omni Amelia Island Plantation Resort is nestled at the tip of a barrier island just off the Northeast Florida coast. Enjoy luxurious oceanfront accommodations, world-class resort pools, championship golf, full-service spa, endless dining options and family-friendly activities. Extend your stay and relax on the largest multi-tiered pool deck in northeast Florida, featuring an adults-only infinity edge pool, a 10,000-square-foot family-friendly pool, Splash Park water playground for the kids, two hot tubs and oversized fire pits. A block of discounted hotel rooms is being held until Monday, October 8, 2018, provided rooms are still available in the STSA room block. After this date, room reservations will be accepted based on availability only.

There are two ways to make reservations:

Room Rate and Cancellation Information:
The rate for a single or double room is $250 per night plus tax, which includes a nightly resort fee covering unlimited high speed internet access, on-property transportation, unlimited use of the Health & Fitness Center, resort beach access, self-parking, local and toll-free calls, and in-room coffee and tea.

Hotel Deposit
One night’s deposit plus tax is required for each reservation. The deposit is refundable on reservations cancelled by 12:00 p.m. three days prior to the date of arrival.

Check-in time is 4:00 pm; check-out is 11:00 am

Please note that you will only be able to book your reservation for the nights of Tuesday, November 6 through Sunday, November 11 if booking
online. If you wish to book an extended stay, making a reservation over the phone is recommended. The resort will honor the group rate up to three days before and after the meeting based on availability.


GETTING AROUND AMELIA ISLAND

The Omni Amelia Island Plantation Resort is an expansive property boasting a long list of activities and attractions for the whole family. The resort provides a complimentary shuttle from 6:00 am to midnight to help you explore all it has to offer. Visit the hotel’s website for a full list of things to do.

Airport Transportation
Jacksonville International Airport (airport code JAX) is located 30 miles from the hotel, and is serviced by most major airlines. The resort does not provide shuttle service from the airport. There are several forms of ground transportation services available at the airport.

Taxi Service
Taxis are readily available at the lower level curbside, outside of baggage claim. Fares to the Omni Amelia Island Plantation Resort are approximately $70 one way.

Shared or Private Car Service
Super Shuttle services the Jacksonville International Airport with both shared rides and private cars. Reservations can be made online at www.SuperShuttle.com. Shared ride cost to the Omni Amelia Island Plantation Resort is $46 per person one way, and private cars are available starting at $86 per person one way.

Private car service is also available through Dana Limousine and Transportation Service. Reservations can be made by calling 904- 744-3333 or emailing reservations@danaslimo.com.

Car Rental
A special car rental rate has been negotiated for STSA meeting attendees through Hertz. For reservations in the U.S. and Canada call 800-654-2240; from all other locations call 405-749-4434. Reservations can also be made online at www.hertz.com. Reference CV# 04R50006 when making reservations.

Parking
The Omni Amelia Island Plantation Resort offers complimentary self-parking for STSA guests. Valet parking is $25 per day.


ACTIVITES AND ENTERTAINMENT

Hospitality Suite

STSA is providing a hospitality room for your spouse or guest to mingle with other spouses and guests and to make plans for exploring Amelia Island.

Residents Reception
Thursday, November 8
Time: 6:00 pm – 7:00 pm
Cost: Complimentary for Residents

Residents and medical students are encouraged to attend this function to meet other residents andstudents, and network with leaders in the field.

President’s Mixer
Thursday, November 8
Time: 7:00 pm – 9:00 pm
Cost: Complimentary

Gather with fellow meeting attendees for an evening of networking and fun. Attendees receive two tickets with registration. Additional tickets may be purchased for $25.00.

Dinner Gala
Friday, November 9
Reception: 7:00 pm – 7:30 pm
Dinner: 7:30 pm- 10:00 pm
Cost:  $125  per adult / $40 per child (ages 12 younger)

Advanced registration recommended. A limited number of tickets will be sold on site.

Join fellow meeting attendees and their families for the always-memorable Dinner Gala, complete with a cocktail reception, dinner, awards, and music entertainment. A local band has been added to this year’s program and will provide easy listening during the dinner hour. Don’t miss it! Although black tie is always in fashion, you are welcome to wear cocktail attire. Be comfortable and have fun!

GOLF AT AMELIA ISLAND’S OAK MARSH GOLF
The Oak Marsh Golf Course is one of the truly classic Pete Dye-designed golf courses in the world. The course, built in 1972 shortly after the completion of the Harbour Town Golf Links at Hilton Head Island, South Carolina, is noted for its tight fairways and small greens, meandering along serpentine salt marsh creeks and through the moss draped heritage oaks for which Amelia Island is famous.

At par 72, this 6,500-yard course has 14 hole swith water hazards and numerous bulk-headed greens. A variety of bunkers are placed throughout the course to add to the challenge, and a natural approach has been maintained as well with the use of coquina shell cart paths, native plant life and preserved habitats for the local wildlife.

Numerous local championships and resort tournaments are played here. Included in Golf Digest’s prestigious list of the “Top 75 Resort Courses in the U.S.,” Oak Marsh also was selected by Travel & Leisure Golf as one of ”Florida’s 50 Finest Courses.”

Individual Golf Play
Cost: $108 (Includes greens fees, cart fees and practice balls.)

Rental clubs are available for $65 and include one sleeve of golf balls. Rental items are not included in the cost of individual golf play.

Please note the following dress code: Men must wear collared shirts with long pants or Bermuda length shorts. Ladies must have a collar or sleeves on their top. Dresses, shorts or skirts of appropriate length are allowed. Only soft-spiked shoes are allowed on the course.


RESORT ACTIVITES AND LOCAL EXCURSIONS

The Omni Amelia Island Plantation Resort features a long list of exciting and rejuvenating on-property activities. On arrival, guests will be provided with a Resort Guide outlining the week’s activity schedule, as well as available excursions and dining options. The resort is situated directly on the beach, and offers two swimming pools, a splash park, full service spa, two fitness centers including state-ofthe art Racquet Park Health Club, a championship golf course, and world-renowned tennis facilities.

Spend your free time exploring the surrounding maritime forests on rented bicycles or Segways, stroll through the Shops of Amelia Island Plantation and pick up souvenirs, or reserve a private cabana on the beach for you and your family. Camp Amelia
Camp Amelia gives guests from ages 4–10 the chance to explore the resort and discover nature, science, salt life, and much more! Interactive learning excursions are offered with full-day themed sessions available. Campers will construct a unique craft and create lifelong memories with an award-winning recreation department. Participants must be fully toilet trained. Pre-registration is required. Camp Amelia is conveniently located adjacent to the pool deck. Evening camp hours are available on Friday and Saturday, 6:00 pm – 9:30 pm. Evening hours may be extended until 10:30 pm for an additional fee of $20 per child.
